How Health IT Can Support ACO Healthcare Quality Improvement

An accountable care organisation (ACO) in Nevada is enhancing healthcare quality across its network of providers with the use of a health IT platform independent of EHRs.

According to Rhonda Hamilton, COO of Silver State ACO, certain provider organisations' quality scores jumped by over 20% within two weeks after the Gaps App from health IT vendor IllumiCare was implemented.

identifying the treatment and coding standards that providers must adhere to for specific patient categories. The EHR workflow for primary care clinicians is directly integrated with these standards via the Gaps App (PCPs).

The technology detects which patient the provider is viewing in their EHR at the point of care and checks the patient's status of care and coding needs. The software gently prods the clinician if there are any care or coding gaps.

As a result, healthcare professionals are better able to identify and address these care gaps during patient visits.

Hamilton said in an interview with EHRIntelligence, "The technology makes it so much easier for the providers to detect and solve those care gaps." Improving patient outcomes will require the ability to fix care gaps and monitor those at the point of service. In the short time we've been utilising it, we've already noticed an improvement in quality scores.

In the post-pandemic environment, she continued, "filling those gaps will require us to ensure that patients are following up on their care and that all preventative actions are being done effectively." Many patients may be reluctant to seek out preventative treatment, so it is important to make sure that the doctors are doing so. "
